Job Description

Registered Practical Nurse - Partners Community Health

Partners Community Health (PCH) is a new not-for-profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH currently operates Camilla Care, a 236-bed long-term care (LTC) home and will soon be operating Wellbrook Place, two new state-of the-art LTC homes and seniors hub in West Mississauga. To be completed in Fall 2023, the new homes will have a combined 632-beds and be part of PCH's larger strategy that will introduce innovative and inclusive programs and services and new models of care delivery. Position Summary:

Reporting to the Director of Care, the Registered Practical Nurse (RPN) is responsible for direct nursing care to residents. This includes providing guidance, direction and support to residents while assuming responsibilities that focus on innovative approaches, quality service and professionalism.

Key Responsibilities:

  • Consistently seeks, learns, applies new and effective ways for establishing good human relations with professional and non-professional staff, residents, families, volunteers, and the community
  • Provides input to the development and revision of Personal Care policies and procedures and communicates changes to staff
  • Facilitates team building and promotes change and innovative ideas.
  • Uses scientific knowledge in a systematic approach to identifying problems and in problem solving
  • Seeks out opportunities to maintain competency and to continuously learn, taking an active role in continuous quality improvement
  • Collaborates with other members of the health team and other disciplines to initiate care plans
  • Assesses, plans, implements and evaluates individualized care plans to meet the needs of residents
  • Assumes responsibility for medication administration, narcotic control, removal of discontinued, unused, or expired medication, application of treatments and dressings, transcription of Physician orders, reordering of medications, preparation, and other tasks
  • Promotes the philosophy of being resident driven in all resident encounters and decisions
  • Promotes and supports the Resident's Bill of Rights
  • Ensures all documentation is timely and accurate while following all organizational policies and procedures
  • Collaborates in the planning and response to change while providing support to other staff and residents where needed
  • Accountable to the DOC for providing nursing care and clinical intervention for the care and services delivered to the Residents on the assigned RHA
  • Provides Functions in accordance with Guidelines for Ethical Behaviour in Nursing
  • Participates in orientation, training of nursing staff as well as Home Area programs (e.g. Occupational Health and Safety Program, Quality Monitoring and Analysis Program, and Fire Safety)
  • Promotes a co-operative working environment
  • Completes other duties as assigned

Qualifications:

  • Nursing degree or diploma required
  • Graduate studies in Geriatrics and Nursing Administration are an asset
  • Current registration with the College of Nurses of Ontario.
  • Current Medication Certificate which meets the College of Nurses Standards of Practice.
  • Members R.P.N.A.O./R.N.A.O./Gerontological Nursing Association preferred.
  • One year experience in a long-term care facility with knowledge and interest in gerontology.
